We are a new type of healthcare company, operating out of Brooklyn and backed by Sidewalk Labs along with some of the top healthcare investors in the country. Our mission is to radically improve the health of urban communities, one block at a time.

In close collaboration with community based organizations and a leading commercial partner, we are reorganizing the health system to focus on what matters to our members. We deliver personalized primary care, behavioral health, and social services through a network of neighborhood hubs with deep community-based partnerships and world-class technology.

Over the next six months we'll expand quickly, growing from 10 to 25 staff in our corporate headquarters, and this role will be a key contributor to the success of the new business.

The role

We are looking for a dynamic and experienced physician-leader to play a key role in care model development and the build of our technology platform, including mapping and hard-wiring clinical data flows and analytics.

You will have experience in building and deploying cutting-edge care delivery models to improve outcomes for individuals with complex healthcare needs, specifically with technology product build: liaising with product, engineering and data science teams to create new digital tools. You will leverage this experience to help us build Commons, our digital care facilitation platform that enables our care teams to connect and coordinate field-based care, perform member-centered care planning, deploy task-oriented interventions and make decisions based on integrated community datasets. You are comfortable sitting at the intersection of the digital and clinical domains and speak both languages fluently.

You will work closely with our Social Care and Product teams as the clinical lead on the build of Commons. This critical role includes codifying key workflow and product specifications, building our analytical and measurement framework, leveraging a variety of data sets (including novel first-party data collected around social influencers of health), providing input around integration with other technical tools, and playing a lead role in end-user testing and product quality assurance, all with the purpose of operationalizing and scaling our unique care model.

You are a clinician, and will practice in our environment, spending up to 20% of your time in direct patient care, and supervising other clinicians - in doing so, your role goes beyond building digital product, and spans to helping Cityblock launch and iterate on it in the real world. As a key member of the clinical leadership team, you must have the drive and ability to inspire and work alongside teams dedicated to caring for patients with complex health and social needs.

You will:

  • Deeply engage with our product and engineering team in defining requirements for Commons, prototyping, testing and bringing the digital platform to life
  • Build role-specific workflows and permissioning for clinical and social care team members
  • Lead product field-testing, incorporate user feedback and refine platform specifications
  • Develop and implement a clinical analytics strategic plan, identifying key outputs of the data model, and supporting the development of comprehensive quality and performance measurement capabilities within the platform
  • Partner with the product team to identify and procure necessary hardware to support the care model platform
  • Partner in building and reinforcing a clinical culture that reflects a commitment to person-centered, integrated and high-quality care delivery
  • Engage in direct patient care, clinical case conferencing and supervision of advance practice clinicians within Cityblock’s care model at approximately 10-20% effort
  • Work with our Chief Health Officer, Chief Product Officer, Social Care, Clinical Operations and Product teams daily.

You’d be a good fit if:

  • You are a primary care physician with 5 - 10 years of progressive leadership experience in health systems innovation, digital product development and quality improvement.
  • You have demonstrated impact as a physician-leader, designing and delivering services in chaotic, complex, value-based environments.
  • You have deep analytical capabilities, an understanding of complex data flows and direct experience parsing and interpreting claims, EHR, and pharmacy data.
  • You have expert knowledge of quality measurement approaches, including Stars, HEDIS and CAHPS - but have also gone beyond to build and track custom-metrics using digital tools.
  • You feel passionately about our mission to transform health and social care for vulnerable populations.
  • You can translate between clinical, technology, business development, operations and policy teams in order to drive collaboration and prioritization.
  • You are rigorous in your approach to rapid cycle innovation and evaluation, and dogged in pursuit of excellent patient and population outcomes.
  • You understand the value of clear and detailed processes, workflows and policies, and have built these from the ground up.
