In close collaboration with community based organizations and a leading commercial partner, we are reorganizing the health system to focus on what matters to our members. We deliver personalized primary care, behavioral health, and social services through a network of neighborhood hubs with deep community-based partnerships and world-class technology.
Over the next six months we'll expand quickly, growing from 10 to 25 staff in our corporate headquarters, and this role will be a key contributor to the success of the new business.
We are looking for a dynamic and experienced physician-leader to play a key role in care model development and the build of our technology platform, including mapping and hard-wiring clinical data flows and analytics.
You will have experience in building and deploying cutting-edge care delivery models to improve outcomes for individuals with complex healthcare needs, specifically with technology product build: liaising with product, engineering and data science teams to create new digital tools. You will leverage this experience to help us build Commons, our digital care facilitation platform that enables our care teams to connect and coordinate field-based care, perform member-centered care planning, deploy task-oriented interventions and make decisions based on integrated community datasets. You are comfortable sitting at the intersection of the digital and clinical domains and speak both languages fluently.
You will work closely with our Social Care and Product teams as the clinical lead on the build of Commons. This critical role includes codifying key workflow and product specifications, building our analytical and measurement framework, leveraging a variety of data sets (including novel first-party data collected around social influencers of health), providing input around integration with other technical tools, and playing a lead role in end-user testing and product quality assurance, all with the purpose of operationalizing and scaling our unique care model.
You are a clinician, and will practice in our environment, spending up to 20% of your time in direct patient care, and supervising other clinicians - in doing so, your role goes beyond building digital product, and spans to helping Cityblock launch and iterate on it in the real world. As a key member of the clinical leadership team, you must have the drive and ability to inspire and work alongside teams dedicated to caring for patients with complex health and social needs.
